Short patient questionaire to help TMC Primary Care access state funds for Health Care Services in the Telluride region.
For Patients who visit the Primary Care department in February there will be a short questionaire of 3 simple questions that you will be asked to answer. Your answers will help TMC Primary Care access state funds that will help offset the cost of offering health care services in the Telluride Hospital District region.
