While it feels like summer has just begun, it’s already time to schedule your kiddos’ annual well child checks before the next school year!* These visits are important to make sure your children are on track with growth and development, immunizations, and developing healthful habits. Also, if your child will be participating in organized athletic activities through the school, they will need a CHSAA pre-participation physical exam and medical eligibility clearance, which is valid for one year from the date of the exam.
In July and August, we have additional appointment slots designated for these important visits. The schedule can fill up quickly, so please call now to get your child’s visit on the books!
Per CHSAA, Parents/Guardians are required to fill out the Medical History Form prior to the appointment. Note: The following are forms were approved by Legislative Council April-2024. No online/virtual physicals should be accepted.
